Great condition, no major flaws or creases. My measurement is not 100% accurate, as the item is not exactly this size to a tee.
This is a low quality photo paper, possibly just the photo was printed at a home printer, then affixed to a cardstock backing, and the autograph was obtained by a collector who loved to get his own photos done. They are a really cool custom made item that this player signed.
